The Philadelphia 76ers will take on the Detroit Pistons on Thursday, November 5, following wins for both teams in their previous outings. Detroit defeated the Charlotte Hornets 124-91, while Philadelphia secured a 126-107 victory over the Chicago Bulls. However, both teams have few people on the injury list. Nevertheless, LeBron James' fitness is another factor heading into the contest, with the veteran forward listed as day-to-day.

LeBron James continues to manage back and sciatica issues while playing for the Philadelphia 76ers

LeBron James is up for Thursday's matchup as he manages lower-back soreness. Despite the ongoing physical concerns, he continues to feature prominently in Philadelphia's rotation during his 24th NBA season. The 41-year-old veteran is coming off a productive performance against Chicago, recording 15 points, seven rebounds, and nine assists.

The forward missed the previous game against the Charlotte Hornets because of lower-back and sciatica management. He was also absent during a scheduled rest period during a back-to-back stretch. He had logged 34 minutes in the preceding game against the New York Knicks, leading the team to manage his playing time rather than risk aggravating his symptoms.

James has also been dealing with lingering nerve irritation and arthritis in his left foot. The 76ers have adopted a treatment and workload management programme to address these issues. He arrives early for physical therapy, deep-tissue treatment and stretching sessions with his trainer, Mike Mancias. Head coach Nick Nurse and the medical staff also monitor his minutes and appearances during back-to-back games to reduce the risk of flare-ups.

LeBron James' history of back problems and the medical explanation behind his sciatica

The back issues first became a concern during the 2025-26 NBA season, when LeBron James was still with the Los Angeles Lakers. He has continued to experience lower-back discomfort and occasional flare-ups since joining Philadelphia. In an interview with Sports Illustrated, Todd J. Albert, surgeon-in-chief at Hospital for Special Surgery, explained the condition and the symptoms associated with sciatica.

"So sciatica is signs and symptoms of irritation of the sciatic nerve. Let's start with what the sciatic nerve is. The sciatic nerve is this big nerve... It's kind of a compilation of three nerves from your spine. So the spinal cord ends right around where your rib cage ends, and then there's a bunch of nerves, and they have numbers; you've heard of them: L1, L2, L3, L4," he explained.

James has used physical therapy, stretching, core-strengthening exercises, and biomechanical training to manage his sciatica. His treatment routine also includes ice therapy and adjustments to his playing schedule when symptoms flare up.
