The Witcher 3 is full of craftsmen and shopkeepers who provide useful services, but there are two in particular that you need to unlock to get the transmogrification system going.

Unlock The Master Craftsmen

Specifically, you need to unlock both Yoanna and Hattori, with the former specializing in armor, while the latter is where you go for all your weapon needs. To unlock them for use, you need to complete both of their associated quests, which in Yoanna's case is Master Armorers, while for Hattori it is Of Swords and Dumplings. To get started, Yoanna is found in the center of Crow's Perch Castle, while Hattori is round the path to the left, if you enter Novigrad via the Glory Gate.

Transmogrify Your Gear

After you have completed their quests, both characters should be willing to help you customize the appearance of your gear. Essentially, you open up their shopfronts as you normally would, though instead of shopping, you'll want to select the Reforge tab. From there, select the piece of equipment that you would like to change the appearance of, select it, and start the reforging process. You'll then be able to select from all of your unlocked appearances, and if you're happy with everything, you can confirm the transaction at the associated cost.

That should leave your character looking very different, while retaining the potency and effectiveness of your favorite weapons and armor.

As mentioned previously, The Witcher 3 Remastered is now available for players to enjoy, and there are far worse ways to spend your week if you're looking for something to do.
