Taylor Swift Turns a Phone Call Into a Thriller for ‘Patient Zero’ Music Video

The cinematic music video stars Dakota Johnson, Colin Farrell, and Cara Delevingne and premiered at the 2026 MTV Video Music Awards

It makes sense that Taylor Swift was plotting how to release the “Patient Zero” music video on a bigger screen. The visual not only features movie stars Dakota Johnson, Colin Farrell, and Cara Delevingne, it’s also a high-production thriller which the superstar premiered at the 2026 MTV Video Music Awards on Sunday.

After Swift was honored at the award show with the inaugural MTV VMA Artist Director Honors, she released the “Patient Zero” music video. In the visual, Swift and Dakota Johnson mirror one another as Johnson struggles through tribulations with Colin Farrell, including a mysterious poisoning. Plot twist: It’s the same playbook that the villainous Farrell pulled on Swift — and it led to her demise.

In a social media post for the premiere, Swift explained the inspiration behind the video’s dark aesthetics. “I got to explore the cinematic and literary genres I love so much. I wanted to incorporate elements of gothic ghost stories, true crime, psychological thrillers all in a setting I hadn’t really shot in much,” she said. “The house was a metaphor for the coldness of a relationship that’s faded and atrophied.”

“I need to figure out a way to get this on TV or something. Google, when are the the VMAs?,” Swift said in a clip from when she wrapped the video that was just shared on social media Sunday morning.

Cinematographer Emmanuel Lubezski, who won Oscars for The Revenant, Birdman, and Gravity, worked on the video. It’s the first time he’s worked with Swift, who previously tapped Rodrigo Prieto for the “The Fate of Ophelia” and “Opalite” music videos from The Life of a Showgirl era.

Last Friday, Swift released The Life of a Showgirl: The Encore with four new bonus tracks, including “Patient Zero” as the lead single. On the song, the singer-songwriter paints the picture of what happens when an ex’s new girl calls you up and asks for advice. Swift told Capital FM that she got the idea after hanging out with her friends and realizing they had all experienced the same thing. “I thought to myself, if this has happened to all of us, it’s such an interesting kind of angle on humanity and on relationships,” she said, adding, “I wonder if I could write something from the perspective of getting this phone call, and really only from the perspective and the side of the phone call of the ex who’s been through it before.”

Swift is nominated in 11 categories at this year’s VMAs, including Video of the Year, Artist of the Year, Best Pop, Best Album, Song of the Summer, Best Direction, Best Art Direction, Best Cinematography, Best Editing, Best Choreography, and Best Visual Effects.
