Chicago Med Season 12’s Mass Casualty Exposes The Cracks In Gaffney's Finest | Review

Chicago Med season 12 opens with an intense mass casualty that sees each of Gaffney's finest pitching in, but exposes the cracks in their foundation.

Chicago Med returned for its twelfth season and, in the midst of a mass casualty event, the opener exposed the cracks of Gaffney Chicago Medical Center's finest as they head into another year. The One Chicago franchise, which has dominated NBC's Wednesday night lineup for the past several years, ended on some explosive notes in the spring, and each finale had its own central focus leading into the fall 2026-27 season. Chicago Med season 11 focused heavily on Hannah Asher's (Jessy Schram) pregnancy, which culminated in an emergency birth during a hospital-wide lockdown, but concluded with the possibility of a massive original cast member exit.

By the end of Chicago Med season 11, S. Epatha Merkerson's Sharon Goodwin's job was on the line as it was revealed that she was being blamed for leaking Dr. Rabari's diagnosis. While viewers weren't sure if she was actually at fault, the final moments of Goodwin's time on screen during Chicago Med season 11's finale made it clear that the storyline wasn't something the series would let go of easily. It wasn't immediately clear if Merkerson would return to Chicago Med for its next installment, but the showrunner quickly dispelled any notion of an exit for Merkerson, instead revealing Sharon would be taking on the fight of her career.

Chicago Med season 12 started off with a bang, in some ways literally, ensuring that every storyline from the previous season was not only addressed, but worked through in some capacity during their intense and action-packed opener.

Chicago Med Season 12, Episode 1 Revisits All Its Finale Cliffhangers

Though Chicago Med season 12 episode 1, "Shots Fired," revisits every storyline it teased out during the finale, they don't all get a clean cut resolution. Instead, the series seems to be pulling the long-game with some of its biggest stories, allowing them to unfold as the season continues. For example, Asher's pregnancy finally came to an end in the last season finale, but the series left things purposefully ambiguous when it came to her romantic relationship with Archer (Steven Weber) despite him sharing his long-simmering feelings. Picking up weeks after the finale, Chicago Med season 12 brings Archer and Asher back together, but the nature of their relationship remains fuzzy as they figure out how to navigate their less-than-traditional family dynamics.

It's clear from the start that the echoes of the finale are still being felt throughout Gaffney's emergency department.

The season premiere also seems set on playing out Sharon's storyline in a longer-game plan, despite the fact that she's back at Gaffney fairly early in the episode for official business. With a rift between Sharon and Dr. Charles (Oliver Platt) that tugs on the heartstrings, Chicago Med appears to be taking the scenic route when it comes to finding Goodwin back behind her desk at Gaffney, but that hardly means that viewers won't get the chance to see her work. In a shocking twist that will certainly come back up as the season continues, the final moments of Chicago Med season 12's premiere shed light into what actually led to Sharon being in hot water with the hospital board – and it's nothing I could've anticipated.

Chicago Med season 12's premiere leaves very little time to check in with Gaffney's finest about their personal lives, but the episode does its best to make sure that everyone is touched upon as they move through a fast-paced trauma after a mass shooting. With each of the doctors and nurses working at a breakneck pace to help the patients coming in, it's clear from the start that the echoes of the finale are still being felt throughout Gaffney's emergency department, and that doesn't resolve itself by the end of the hour. Getting back to the status quo is likely more of a season-long endeavor for Chicago Med, but having the opportunity to check in makes a difference for the opening episode.

Chicago Med Season 12 Is Sticking To Its Best Elements

Although Chicago Med season 12's premiere does touch upon a lot of the more personal storylines that played out in the finale – including one exciting surprise that I didn't expect by episode's end – the show ultimately does what it does best throughout the episode by focusing on its cases. The mass casualty, a shooting that disrupts a wedding in more ways than one, brings in several compelling cases that allow many different doctors to jump in and show their expertise. From Asher jumping in to help a patient after simply stopping by for a visit with baby Mabel to Frost (Darren Barnet) and Ripley (Luke Mitchell) working together on a heartbreaking case, Chicago Med moves quickly and grounds the episode with medicine rather than the interpersonal drama looming large.

Each of the patients that Chicago Med chooses to spend time on throughout the episode deliver, though some make more of an impact on the story as a whole than others. A standout guest appearance from Gabrielle Carteris anchors one of the hour's most interesting stories, but by the end of shift, things have settled considerably from the mass casualty, despite a surprising third act twist that's more crushing than I anticipated.

With two new faces joining the series in Dr. Annie Wu (An-li Bogan) and Mallory Sloane (Rebecca Breeds), Chicago Med has a lot of ground to cover over the course of its first hour of the season. Dr. Wu, who comes into play quickly and makes quite the impression throughout the episode, is a standout throughout the premiere, and definitely someone to keep an eye on as the season continues. While there are still quite a few unanswered questions surrounding what's happening at Gaffney, both within the internal structure of the hospital and within the doctors' lives themselves, Chicago Med season 12's premiere set the tone for an exciting season ahead.

Chicago Med season 12 airs new episodes every Wednesday at 8 p.m. ET on NBC.
